    NOTICE: Health Assessment Dates

    Hello,

    COH has pushed the start of this year's Health Assessment from August 1, 2018 to May 1, 2018. This means it has already opened.

    You can start completing the wellness program and earning your medical plan rate discount today! You may obtain your biometric numbers from your physician during annual preventative exam or attend one of our upcoming biometric screening events. Once you have your biometric numbers, complete your online health assessment by November 30, 2018, and you have until March 8, 2019, to complete three wellness options. The health assessment and the reporting of your wellness options are done online at mycigna.com.

    NOTICE: Houston's Healthiest Employer Award

    From the City of Houston's Human Resources Department:

    The City of Houston Health and Wellness Program is honored to be selected as a Finalist in the Houston Business Journal's Houston Healthiest Employer Award in the category for "Extra Large Employers" (5,000 or more total employees). We receive this designation with other distinguished employers such as Baylor College of Medicine, BP, Chevron, Harris Health System and more.

    We could not have achieved this designation without the hard work of our employees like you who improved their overall health and wellbeing by participating in our various Health and Wellness Programs.

    To thank our employees for helping us to reach this designation, the City of Houston has awarded all City of Houston Employees who completed the entire 2017-2018 Wellness Program (online health assessment and one wellness engagement option) with a Cigna Healthy Awards Account in the amount of $35.

    Employees may utilize these funds for medical services such as physician visit copays, pharmacy claims, and inpatient/outpatient hospital facilities. Funds will expire April 30, 2019, so employees must utilize them prior to this date or the funds will be forfeited.

New Statistics Sheets

Hello,

Please be aware that the program and outreach logs have been moved from Google to a third-party application, Typeform. Unfortunately, I have yet to find a way to upload and/or publish results in real time in a sheet and/or page outside of Google - that is a work in progress. 

Until then, the results will be sent to Google Spreadsheets until further notice. 

Finally, there are no secondary forms for HEAR statistics: programs are to be entered into the program log for the host location, and all outreaches are to be logged in the outreach form. The reason for the separate location forms is that Typeform only allows 100 submissions per month. Since both locations are doing, on average, about 30+ programs in a month, having two forms gives us a bit of breathing room in regards to being able to log all programming statistics.

Remember: stats are due on the 1st of the month for the multi-service centers, and on the 7th of the month for HPL. Please be diligent in recording your statistics in a timely manner so that all stats can be reported on time.

CMC Committee Lead Coordinator and Team Positions

Greetings HPL Team,

The city’s annual Combined Municipal Campaign (CMC) is just around the corner! Each year, the HPL Team rises to the occasion with wonderful activities, stand-out participation and most importantly, fundraising to support the CMC organizations and their very important missions. In preparation for the 2019 “We Rise by Lifting Others” campaign we are seeking HPL committee members for the following CMC lead roles:

CMC Committee Positions
Lead Coordinator: The Lead Coordinator will act as a representative for HPL’s CMC volunteer committee. Duties include: training and managing volunteers and fundraisers, tracking donations, and ensuring proper documentation of pledges and overall management of the department’s campaign. The lead coordinator will work with the director in setting contribution and participation goals for the department.
Co-Coordinator: The co-coordinator assists the lead coordinator and acts as a stand-in if the Lead Coordinator is unavailable. Duties include: acting as back up for the Lead Coordinator and assisting with Lead Coordinator duties.
Secretary: Duties include: scheduling fundraising and agency events on the online CMC event calendar, as well scheduling large-scale events with CMC Chair, Jennifer Cobb; scheduling agencies for departmental agency fairs and disseminating information about that agency to the department, taking photos at department events, and maintaining department documentation.
Treasurer: Duties include: management of collected donations, management of fundraiser monies, maintenance of Excel reporting workbook and donation reports, and reconciliation of all funds.

The schedule of CMC events is as follows:
  • Coordinator Training & Executive Meeting – Tuesday, August 21 from 8:30am-10:30am
  • Agency Fair – September
  • Online Auction – October 8-19
  • Reconciliation – November 5-16
  • Recognition – February 2019

New Statistics Sheet

Hello,

Starting in FY19 (which began on July 1st), we will be collection statistics for Computer Help 1-on-1. Computer Help 1-on-1 statistics is meant to capture how many times staff are assisting customers in computer/digital literacy. Much like the language stats, you would document these encounters twice: once under the Computer One-on-One Column and then again under the Reference Questions column on the statistics sheet.

Below is a longer explanation of the new statistics provided by Government Affairs Liaison Mary Hammond:

Computer Help - 1-on-1: This purpose of this category is to capture how many times we are assisting customers in computer/digital literacy. These encounters will also always be counted under "Reference Questions".
  • "Computer Help 1-on-1" involves the "how": How do I log on? How do I print? How do I download? How do I set-up a username? How do I set-up an account? This type of question is recorded as "Computer Help 1-on-1" and "reference question".
  • Computer-related reference questions would be those associated with "where" or "what": What's a good site for recipes? Where's the online Houston Chronicle? Where's information on genealogy? This type of question is recorded as a "reference question" only.
Example: A customer comes in wanting to know how to check on their tax return and they don't know how to do a search for the IRS.
  • Showing them how to do an Internet search is a computer/digital literacy interaction ("Computer Help - 1-on-1" count) and a reference question.
  • Showing them how to set up an account (i.e., specific account, email) is a computer/digital literacy interaction ("Computer Help 1-on-1" count) and a reference question.
  • Informing them about Internet safety is a reference question but not a "Computer Help 1-on-1" count.
  • Advising them of where to search for tax information is a reference question but not a "Computer Help 1-on-1" count.
